Cityblock Health Integrates Primary Care, Behavioral Health, and Social Services. It has developed a technology platform called Commons to facilitate collaboration among care providers. This tool enables Cityblock to deliver a new model of care where Community Health Partners (CHPs), hired from the community and trained in empathy and relationship-building, can serve patients. The Commons technology then places the CHP at the center of the care and communication team.If a patient has an issue, the CHP can assist in navigating or reporting the problem. The platform is also designed for information sharing. It connects medical records, pharmacy claims, community health resources, and interactions with care team providers. The platform also features a clinical decision support tool to help personalize care and action plans for patients.
