ThoughtSwift Medical Assessments is an early medical software provider offering medical assessments for primary care and internal medicine physicians. The program collects key psychological information related to the patient's current mental and psychological condition by having them answer a series of questions on an iPad or computer. It immediately evaluates (but does not diagnose) the patient’s health status and overall mental state. The second project is the Annual Health Report, which gathers information and generates a report that doctors should use to create a five-year chronic care plan for elderly Medicare patients. The third project is the Chronic Care Tracking Program, which, like all company initiatives, is a standard care component for all physicians seeing patients. Each elderly patient can receive one such report annually, while commercially insured patients under 65 can obtain an annual psychological assessment once a year.
