MyHealthTeams, Founded in 2012, is a social network for people living with chronic conditions, providing services such as condition sharing, diagnosis and treatment discussions, advice, and encouragement of mutual support. We believe that when individuals facing the same chronic conditions can communicate and learn from each other—asking questions, receiving recommendations, and sharing insights about coping with the disease—health outcomes improve. You are not alone, and there’s no need to reinvent the wheel. Within a few years, we believe that if you are diagnosed with a chronic condition, your doctor will prescribe a social network for you.
