MHN ACO is owned and operated by 16 nonprofit healthcare organizations in Cook County, including 13 federally qualified health centers and 3 hospitals. MHN ACO is recognized as one of the best-performing Medicaid ACOs in the United States and was the first entity in Illinois to receive NCQA case management accreditation. Since its establishment in 2014, we have been committed to improving the health of underserved communities through innovation and collaboration. MHN ACO invests in personnel, communities, and provider organizations to transform delivery methods and enhance overall health equity.
