LTC ACO

LTC ACO is the first accountable care organization to provide services for Medicare beneficiaries residing in long-term care facilities. Since 2016, LTC ACO has specifically focused on the unique needs of frail elderly individuals, those suffering from Alzheimer's disease or dementia, depression, amputation, developmental or physical disabilities, renal failure, as well as those with multiple chronic conditions and complications.
