24 Our Care, LLC is a U.S.-based company that provides value-based, non-face-to-face monthly care management services to patients through its care coaches, working under the comprehensive supervision of the patients' healthcare providers. The care coaches connect directly with patients monthly via phone or other electronic means, outside of regular outpatient visits, to offer eligible patients monthly chronic care management, primary care management, behavioral health integration, remote patient monitoring, advanced care planning, health risk assessments (for healthcare providers to use during patients' annual wellness checks), and transitional care management services. These services improve patients' lives by enhancing their overall health and daily healthcare while reducing the number of medical visits.
