PREPARE

PREPARELaunched by UCSF in 2013, incorporating feedback from patients and caregivers of diverse cultural backgrounds, PREPARE is a free bilingual online program that uses plain language and "how-to" video examples to guide users through five planning steps for medical decision-making. It focuses on helping users explore their wishes and learn how to discuss these wishes with family, friends, and healthcare providers. PREPARE also offers tools to create easy-to-read, legally binding advance directives for all 50 states in English, Spanish, and an increasing number of Chinese languages. Additionally, resources are available for healthcare professionals and community organizations. PREPARE is the only person-centered advance care planning program designated as evidence-based by the National Council on Aging.
