West PACE

West PACE is a program that provides all care and services for seniors aged 55 and over living in North County San Diego and those who meet the state's nursing home level requirements. Our mission is to enable seniors to live as independently as possible in their communities by offering compassionate, comprehensive health, and supportive care.
