My Choice Wisconsin is a Managed Care Organization (MCO) and Health Maintenance Organization (HMO) located in Wisconsin, with the goal and capability to provide seamless care. My Choice Wisconsin builds relationships with a diverse provider network and community partners to offer members the best possible choices and services. Through five programs in Wisconsin (Family Care, Partnership, Medicaid SSI, BadgerCare Plus, and Medicare Dual Advantage), the company's members gain access to long-term care and health services, helping them live their best lives independently within their own communities.
