As an accountable care organization, Catalyst Health Network improves health outcomes, coordinates care, and reduces costs—creating sustainable and predictable value. The network consists solely of primary care physician members and is managed by StratiFi Health, a physician services and population health organization that provides integrated solutions to support and optimize the doctor-patient relationship and proactively shape the patient’s healthcare experience.
