Through Our Combination of the agilon Platform, Long-Term Partnership Model with Existing Physician Groups, and a Growing Network of Like-Minded Physicians, We Are Poised to Deliver Revolutionary Healthcare Services to Seniors Across Communities in the United States. Our Purpose-Built Model Provides Existing Physician Teams with the Necessary Capabilities, Capital, and Business Model to Create a Medicare-Focused, Global Capitation-Based Line of Business. Our Model Operates by Establishing Risk-Bearing Entities (Each Referred to as an “RBE”) Within Local Regions, Which Enter Into Agreements With Payers to Receive Monthly Payments for Managing the Total Healthcare Needs of Patients Attributed to Our Physician Partners (Or Global Capitation Agreements). These RBEs Contract With agilon to Perform Certain Functions and Enter Into Long-Term Professional Services Agreements With One or More Primary Care Physician Groups. Under These Agreements, the Primary Care Physician Groups Receive a Base Compensation Rate and Share in Savings Generated From Successfully Improving Care Quality and Reducing Costs.
