
Eli Lilly's Moves in the Inflammatory Bowel Disease (IBD) Arena Continue Unabated. On February 25, Organovo Holdings, Inc. announced that Eli Lilly and Company will acquire Organovo's FXR program, including its core asset FXR314. Organovo is a biotechnology company focused on developing treatments for inflammatory bowel disease (IBD) using innovative approaches.According to the agreement, Organovo will receive an upfront payment and collect milestone payments upon FXR314 achieving key regulatory and commercialization milestones. Eli Lilly and Company will obtain global development and commercialization rights for Organovo's FXR project, including all intellectual property.FXR314 is a farnesoid X receptor (FXR) agonist. FXR, as a nuclear hormone receptor, is crucial for maintaining bile acid, lipid metabolism, and intestinal homeostasis. In fact, Organovo's initial lead indication for advancing FXR314 was metabolic dysfunction-associated steatohepatitis (MASH) rather than IBD.In April 2024, Organovo announced the Phase II study results of FXR314 for the treatment of MASH. The results showed that, compared with the placebo group, the liver fat content of patients in the FXR314 group significantly decreased from baseline. Additionally, the degree of liver fibrosis in the FXR314 group did not worsen. However, another FXR agonist, obeticholic acid, also attempted to target the MASH indication but ultimately failed.Organovo believes that the acquisition opportunity for FXR314 still lies in the IBD sector. First, the market potential of the IBD sector is enormous. According to Organovo, by 2032, the IBD market size could reach hundreds of billions of dollars. Second, many powerful pharmaceutical giants have a strong interest in immunology, and their product pipelines are aging, urgently needing an injection of new vitality. Additionally, a move by Eli Lilly has also clarified the opportunity for Organovo.In July 2024, Eli Lilly and Company acquired Morphic Therapeutic for $3.2 billion with the aim of strengthening its IBD portfolio. This acquisition was prominently highlighted in Organovo Holdings, Inc.'s presentation slides, indicating a growing trend of transactions in the IBD space.The core of acquiring Morphic was MORF-057, an oral small-molecule α4β7 inhibitor in Phase II clinical trials. Takeda's α4β7 monoclonal antibody vedolizumab, which targets the same pathway, had already set a successful precedent, becoming a blockbuster drug with sales of $5 billion driven by its penetration in IBD indications.Targeting the IBD market, Eli Lilly has launched the IL-23 monoclonal antibody Mirikizumab. The drug entered the market in 2023 and has been approved for two indications: ulcerative colitis and Crohn's disease. Mirikizumab boasts impressive results, having outperformed Johnson & Johnson's ustekinumab in the Phase III VIVID-1 study for Crohn’s disease treatment, demonstrating better histological remission.So, why did Eli Lilly decide to take action on FXR314? Organovo is committed to developing candidate drugs validated for efficacy based on three-dimensional (3D) human tissue, and FXR314 represents a significant milestone achieved by Organovo in advancing IBD drug research using proprietary 3D human tissue models.According to the introduction, FXR314 has a similar effect on colitis improvement as approved IBD treatments. In preclinical models of adoptive T-cell transfer, FXR314 demonstrated dose-dependent improvement in indicators related to ulcerative colitis.In Organovo's plan, patient enrollment for the Phase IIa clinical trial of FXR314 in treating ulcerative colitis has commenced in Q1 2025, with related results expected to be released in H1 2026.In addition, Organovo also disclosed that there is another target molecule in the FXR project, which started drug chemistry research in 2023.
