Recently, the "Expert Consensus on Glucagon-Like Peptide-1 Receptor Agonists Combined with Insulin for the Treatment of Type 2 Diabetes (2025 Edition)" compiled by the Diabetes Specialty Committee of the Chinese Research Hospital Association was published in the Chinese Journal of Diabetes. The Hansoh Pharma's PEGylated Losenatide Injection (brand name: Fulaimi) has been successfully included in the recommendations, contributing to the innovation in combined therapy for type 2 diabetes.

In China, insulin is widely used in the treatment of diabetes, but hypoglycemia and weight gain are common and non-negligible adverse reactions. The combination of glucagon-like peptide-1 receptor agonists (GLP-1RA) with insulin can intervene in multiple pathophysiological defects of type 2 diabetes (T2DM). This not only significantly enhances the hypoglycemic effect and reduces daily insulin dosage but also mitigates the risks of weight gain and hypoglycemia, making it a commonly used combination therapy in clinical practice. The "Expert Consensus on Glucagon-Like Peptide-1 Receptor Agonist Combined with Insulin for the Treatment of Type 2 Diabetes (2025 Edition)" aims to standardize the clinical application of this combination therapy and provide grassroots physicians with scientific and authoritative references and guidance.

The consensus points out that an adequate dose of GLP-1RA combined with insulin can bring additional benefits such as cardiovascular and renal protection to patients. Both daily and weekly formulations can significantly reduce glycated hemoglobin (HbA1c) levels and improve the rate of blood glucose control. Moreover, T2DM classification based on fasting C-peptide provides a convenient approach for precision treatment, with GLP-1RA adapting to precise treatment plans for different T2DM subtypes.

Fu Laimei is China's first self-developed GLP-1RA weekly formulation and the world's first PEGylated long-acting hypoglycemic drug, which was quickly included in the national medical insurance catalog after being approved for marketing in 2019. Clinical studies have confirmed that Fu Laimei has a clear hypoglycemic effect, plays a positive role in weight loss, improving blood lipids and blood pressure, and protecting the kidneys, with good safety. Its once-weekly subcutaneous injection method greatly enhances patient convenience and compliance.

Diabetes Therapy recently published a retrospective real-world study, which for the first time evaluated the efficacy and safety of Fulaimi in combination with basal insulin. The study confirmed that this combination therapy not only optimizes blood glucose control but also provides additional weight loss benefits, especially showing more significant effects in obese patients, female patients, and those with longer disease duration, without significantly increasing the risk of hypoglycemic events. This study strongly supports the clinical application of Fulaimi combined with insulin, offering a new treatment option for T2DM patients who fail to achieve glycemic control targets with insulin alone.

Previously, Fulaimi has been included in the "China Diabetes Prevention and Treatment Guidelines (2024 Edition)" formulated by the Chinese Diabetes Society (CDS) of the Chinese Medical Association. This inclusion in the expert consensus on GLP-1RA combined with insulin therapy for T2DM will further enhance the scientific and standardized clinical application, bringing more precise treatment to patients with T2DM.
