Patient Information
Elderly female, 66 years old, admitted to the hospital with "paroxysmal palpitations for 1 year", diagnosed with: paroxysmal atrial fibrillation (CHA2DS2-VASc score 3; HAS-BLED score 3). After comprehensive preoperative evaluation, valvular heart disease was ruled out, no left atrial thrombus was found, and the patient met the indications for pulsed electric field ablation of atrial fibrillation. After full communication with the patient and her family, it was decided to useShineyoMedical RhythPulse®PFA Pulsed Electric Field AblationTreat the patient.

Surgical Procedure
The patient was placed in a supine position, routinely disinfected, and draped. The puncture site was locally anesthetized with 2% lidocaine. After two attempts at puncturing the right femoral vein, a coronary sinus electrode was inserted. Following successful transseptal puncture, contrast agent was injected to confirm that the puncture needle was located in the left atrium. Heparin was administered systemically, and under the guidance of a three-dimensional mapping system, modeling of the left atrium and pulmonary veins was performed. Under analgesic sedation with sufentanil combined with midazolam, pulsed-field ablation was conducted for bilateral pulmonary vein electrical isolation. During the isolation of the posterior wall of the right pulmonary vein, atrial fibrillation converted to sinus rhythm. Bilateral pulmonary vein electrical isolation was completed, and both atrial input and pulmonary vein output block were verified.
