The successful completion of the surgery not only marks the official entry of this innovative domestically-produced PFA system into the clinical application stage, but also, with its three core advantages of integrated circular catheter mapping and modeling, efficient ablation, and excellent safety, provides a better option for atrial fibrillation interventional treatment.

Substrate Mapping Before and After Ablation

The patient in this surgery was a 79-year-old female with recurrent atrial fibrillation after radiofrequency ablation. Based on preoperative examinations, the anteroposterior diameter of the left atrium was 46mm. An AccuPulse 20-25mm adjustable-loop cardiac pulse ablation catheter was selected for the ablation. The patient was in a state of atrial fibrillation at the start of the procedure. The total duration of the ablation surgery was only 40 minutes, including 1 minute for catheter modeling and mapping, 24 minutes for pulmonary vein ablation, and a total discharge time of 19.6 seconds. After the ablation, a 29mm occluder was successfully used to perform left atrial appendage closure.

First, establish left atrial access through venous puncture and atrial septal puncture. After inserting the pulsed field ablation (PFA) catheter, perform three-dimensional modeling of the left atrium and substrate mapping. Mapping revealed high-voltage areas at the ostium of the right superior pulmonary vein and the region between the two anterior pulmonary veins on the left. During ablation, a total of 12 pulses were delivered at the ostium of the right superior pulmonary vein; 16 pulses for the left superior pulmonary vein; and 15 pulses for the left inferior pulmonary vein. Given that the patient had recurrent persistent atrial fibrillation, an additional BOX linear ablation was performed on the posterior wall of the left atrium, delivering a total of 69 pulses. Voltage mapping was repeated after ablation to verify whether the pulmonary vein isolation and posterior wall BOX ablation were complete and continuous.

Expert Commentary

This surgery was for recurrent atrial fibrillation, and the procedure adopted a verification and supplementary ablation approach based on the recovery of pulmonary vein potentials.
