Recently, the electrophysiology team of the Cardiology Department at the Affiliated Hospital of Zunyi Medical University successfully completed one of the first cases in China using the domestically produced ShineyoMedical RhythPulse under ICE guidance.®PFA Pulsed Field Ablation Surgery. The validation of PFA technology's comprehensive advantages in effectiveness, safety, and efficiency; marks that China's indigenous PFA technology is developing comprehensively, providing a safer and more efficient new treatment option for patients suffering from atrial fibrillation.

Patient Information
Patient: Female, 57 years old
Chief Complaint: Palpitations for 1 Year, Recurrent for 20 Days
Present Illness: A 57-year-old female patient was admitted to the hospital due to "palpitations for 1 year, recurring for 20 days." Over the past year, the patient has repeatedly experienced palpitations accompanied by dizziness and amaurosis, but without nausea or vomiting, and no chest tightness or chest pain. For the past 20 days, the patient's palpitation symptoms have occurred more frequently with prolonged duration, lasting from a few minutes to about ten minutes each time. She then visited the local hospital where further examinations suggested "atrial fibrillation." For further diagnosis and treatment of atrial fibrillation, she visited our hospital’s outpatient department and was admitted to our department with a diagnosis of "paroxysmal atrial fibrillation." Since the onset of the disease, her mental state and sleep have been fine, appetite is normal, bowel movements and urination are regular, and there has been no significant change in weight.
Preoperative Diagnosis: Paroxysmal Atrial Fibrillation

Communicate with the patient and family members. The current diagnosis is paroxysmal atrial fibrillation, which indicates the need for catheter ablation. Generally, traditional radiofrequency ablation is chosen, but the newer pulsed field ablation for atrial fibrillation is also an option. After fully informing them of the pros and cons of both procedures, the patient and family decided to choose ShineyoMedical RhythPulse.®Treatment with PFA pulsed electric field ablation.
Surgical Procedure

During the pulse atrial fibrillation surgery under deep sedation, the patient lies supine, with routine disinfection and draping. The puncture site is locally anesthetized with 2% lidocaine. Under ultrasound guidance, three punctures are made in the right femoral vein to insert the coronary sinus electrode and the AgileView ICE intracardiac echocardiography catheter. After successful transseptal puncture guided by ICE, heparin is administered for systemic anticoagulation. ShineyoMedical's RhythPulse is selected.® A 10mm ring-shaped pulsed electric field ablation catheter was used for three-dimensional pulsed electric field ablation, with continuous ICE monitoring of the catheter position, contact, pericardial conditions, and thrombosis during the procedure.

ICE-guided PFA Catheter in the Superior Vena Cava
The patient underwent left-sided PVI while in sinus rhythm. Atrial fibrillation (AF) occurred during the ablation of the left inferior pulmonary vein. After completing bilateral PVI, AF had not terminated, so additional ablation lines were applied to the posterior box, superior vena cava, and tricuspid isthmus; however, AF still did not terminate. Subsequently, Marshall vein ethanol ablation was performed, during which AF converted to atrial tachycardia (AT). The A-wave at CS 9-0 was the earliest, suggesting a right atrial origin. RhythPulse was then used.®️-PFA catheter was withdrawn to the right atrium, and activation mapping showed an origin from the superior vena cava. Focal ablation was performed below the original isolation line at the septal side of the superior vena cava, after which sinus rhythm was restored. Upon completion of the procedure, electrophysiological examination did not induce any other arrhythmias. Bidirectional conduction block was achieved in both pulmonary veins and the isolation lines. ICE showed no pericardial effusion or thrombus, and the surgery was successfully completed.
