Large Pediatric Patient Population, Scarce Rehabilitation Expertise and Institutions


China has a pediatric population of 256.5 million. The field of pediatric rehabilitation is characterized by high treatment costs and a scarcity of specialized institutions, with cerebral palsy, autism spectrum disorder, and developmental delays being the three predominant conditions that continue to afflict hundreds of millions of families.


Currently, the incidence of cerebral palsy in children among newborns is approximately 5‰. There are about 6 million individuals with cerebral palsy in China, and it is projected that around 1 million new cases will emerge over the next decade. When including patients with sequelae of traumatic brain injury and encephalitis, as well as those with peripheral nerve injuries, the total number is expected to reach approximately 10 million.

 

The prevalence of autism is approximately 1%, with more than 10 million affected individuals. Among them, there are about 6 million children under the age of 14. Even by conservative estimates, the number of patients exceeds 2 million, with approximately 200,000 new cases each year.

 

According to statistics, the prevalence of growth retardation among children in China is as high as 9.9%, ranking second globally. The total number of children aged 4–15 with growth retardation in China is approximately 7 million. Each year, very few children receive appropriate treatment; notably, 97% of parents are unaware of the basic principles of child growth and development, while 30% remain blindly optimistic, clinging to outdated beliefs such as “late bloomers.”


Compared with adult rehabilitation, pediatric rehabilitation involves greater technical and managerial complexities and higher entry barriers, resulting in a severe shortage of medical institutions equipped with pediatric rehabilitation capabilities.


Taking tertiary hospitals as an example, these institutions prioritize the treatment of acute and critical conditions. Their rehabilitation departments are often transformed from traditional Chinese medicine or physiotherapy units, resulting in insufficient awareness and emphasis on pediatric rehabilitation. Moreover, general rehabilitation requires prolonged courses, with hospital stays lasting three to six months or even up to a year; certain pediatric conditions necessitate lifelong rehabilitation. Given the scarcity of beds in public hospitals, this places significant pressure on bed turnover rates and poses substantial challenges to hospital service delivery.


Currently, service institutions under the civil affairs departments and the China Disabled Persons' Federation system cover rehabilitation services for the majority of children with disabilities and more than half of those with severe conditions. In contrast, private pediatric rehabilitation institutions primarily focus on education and manual therapies, and largely lack medical practice licenses.


Meanwhile, the significant shortage of medical experts in pediatric rehabilitation imposes greater limitations on treatment approaches compared to clinical interventions available in medical institutions. As a result, care is often restricted to non-invasive modalities such as physical therapy and psychological intervention, which still lag behind the comprehensive treatment options accessible within medical institutions.
