Prosthetic Joint Infection (PJI)

In the United States, more than 40,000 people develop prosthetic joint infections (PJI) each year, a rare but potentially devastating complication following joint replacement surgery. After infecting the prosthetic joint, pathogenic bacteria form biofilm structures that are difficult to eradicate, necessitating prolonged, invasive, and costly treatments. However, these interventions often fail, resulting in high rates of permanent disability and mortality. Recent clinical evidence indicates that the current gold standard for treating PJI—two-stage revision arthroplasty—requires an average of 16 weeks and achieves a success rate of less than 50% at 12 months. This underscores the urgent and unmet clinical need for more effective management of post-arthroplasty infections, highlighting the demand for faster and more reliable therapeutic solutions.

About VT-X7

VT-X7 (local irrigation with vancomycin and tobramycin) is an innovative drug-device combination therapy for periprosthetic joint infection (PJI). It delivers high concentrations of mature, broad-spectrum antibiotics—vancomycin and tobramycin—directly into the joint cavity and surrounding tissues affected by postoperative PJI. VT-X7 is a 7-day treatment regimen designed to provide rapid and reliable management of rare but highly threatening infections, thereby addressing an unmet clinical need. In completed Phase II clinical trials, all patients undergoing two-stage revision arthroplasty who received VT-X7 proceeded to reimplantation of the second-stage prosthesis just 7 days after the first-stage explantation and debridement, with 93% of patients remaining infection-free at one year.

Osteal is currently advancing the APEX study, a randomized controlled trial comparing a 7-day VT-X7 regimen for chronic periprosthetic joint infection (PJI) against the current gold standard—two-stage revision arthroplasty over 16 weeks. The FDA has granted VT-X7 Orphan Drug, Fast Track, and Qualified Infectious Disease Product (QIDP) designations. These recognitions underscore a first-in-class, multi-billion-dollar market opportunity. More importantly, VT-X7 has the potential to significantly improve therapeutic outcomes for patients with post-arthroplasty infections who currently respond poorly to existing treatments.

About Osteal Therapeutics, Inc.

Osteal is a clinical-stage biopharmaceutical company dedicated to developing innovative products for the treatment of infections following artificial joint replacement. The company employs high-concentration, locally delivered antibiotics to treat biofilms formed in prosthetic joint infections (PJI). This approach not only enables rapid resolution of infection but also minimizes systemic side effects on other organs associated with high-dose systemic antibiotic use. Osteal adopts a low-risk development strategy by innovatively repurposing approved drugs with established long-term safety and efficacy profiles for local administration. The company’s lead product, VT-X7, is currently in Phase II clinical development for the treatment of PJI, a serious complication following joint replacement surgery.
