Professor Zhu Da (Yunnan Fuwai Cardiovascular Hospital)It was pointed out that the proportion of moderate or higher MR combined with TR in the Chinese population is as high as 26.92%, and the prognosis of such patients is worse. Studies have shown that performing M-TEER + T-TEER simultaneously can significantly reduce the 1-year all-cause mortality rate. Confirming leaflet capture volume and tissue bridge stability through multi-planar views is key to ensuring procedural success.

Professor Song Sixian (Taipei Veterans General Hospital): For wide regurgitant jets or complex lesions, a single clip often struggles to achieve satisfactory regurgitation control; in such cases, the "dual-clip strategy" is crucial. Professor Song emphasized that the placement of the second clip must follow the principles of "parallel, close, and complementary," ensuring the tissue bridge formed by the two clips is stable and does not cause valve orifice stenosis through precise adjustments in orientation. The flexible maneuverability of the NeoNova® system provides strong support for the accurate positioning of the dual clips, aiding surgeons in achieving more thorough anatomical repair.

Professor Feicheng Yu (Shao Yifu Hospital, School of Medicine, Zhejiang University)Shared a highly challenging "One Sheath, Two Valves" case. The patient had previously undergone tricuspid annuloplasty and was admitted this time due to MR 4+ combined with TR 5+. During the procedure, after a single transseptal puncture and application of the NeoNova® system, regurgitation significantly decreased and cardiac function greatly improved. This case fully demonstrates the flexibility and efficiency of the NeoNova® system in treating complex combined lesions.

• Technical Challenge: The patient's LVEF was only 28.8%, indicating a very low cardiac function status.

• Surgical Highlights: The NeoNova® system was applied during the surgery. With its ingenious design and stable control performance, two clamps were successfully implanted under low cardiac function conditions, achieving precise capture and efficient repair. Postoperative regurgitation significantly improved, fully verifying the safety and effectiveness of NeoNova® in high-risk patients with low cardiac function.

• Technical Challenge: The reflux involves a relatively wide range (approximately 23mm), and the patient has multiple underlying conditions.

• Surgical Highlights: During the operation, the NeoNova® system demonstrated flexible capture ability and clipping mechanism under precise guidance of 3D and X-plane ultrasound. After the implantation of two C6 clamps, ideal edge-to-edge repair was achieved within a wide regurgitation area, and the postoperative transvalvular pressure gradient was well controlled.
