With the continuous development of interventional treatment technology for lower extremity arteriosclerosis obliterans (ASO), the strategies for managing complex lesions are becoming increasingly refined. Particularly when dealing with challenging lesions such as calcification, in-stent restenosis, and intimal hyperplasia, conventional balloon dilation tends to cause complications like excessive vascular injury, flow-limiting dissections, and hematomas. In recent years, directional scoring balloons, as a new type of pressure-focused balloon, have demonstrated potential to effectively dilate lesions while reducing vascular injury, minimizing dissection risks, and optimizing luminal gain due to their innovative design.

DKutting® LL adopts DK Medtech's unique patented triangular nitinol coil technology, combined with the actual clinical needs of lower limb arteries. It significantly increases the length of the scoring element (up to 150mm) and develops a new fully coaxial balloon delivery system compatible with 0.014"/0.018" guidewires, while enriching the selection of balloon diameters at 0.5mm intervals.

Case Sharing

Italian peripheral intervention expert Dr. Mariano Palena shared a complex case involving multi-segment lesions of the above-the-knee artery (ATK), femoral-popliteal artery (Fem-Pop), below-the-knee artery (BTK), and below-the-ankle artery (BTA). The DKutting® LL (2.5*150mm/3.5*120mm) successfully opened the below-the-ankle artery and restored blood flow.

This case of ATK→Fem-Pop→BTK→BTA full-limb multilevel PAD lesions can be considered a "top-tier" complex case in the field of peripheral arterial intervention. The lesions span multiple anatomical segments with significant variations in vessel diameter and calcification levels, making CTO lesion recanalization extremely challenging. Dr. Mariano Palena successfully recanalized the lesions by utilizing retrograde access techniques along with various devices. In this case, DKutting® LL demonstrated excellent bending capability and powerful expansion performance.
