Introduction

The latest 《Guidelines for the Diagnosis and Treatment of Primary Liver Cancer (2024 Edition)》 recommend transarterial chemoembolization (TACE) as the first-line treatment for patients with CNLC stage IIa, IIIa, and partial stage IIIb. Similarly, international BCLC staging guidelines also recommend TACE as the first-line treatment for patients with BCLC stage B.

Patient Case Details



Basic Information: 46-year-old maleSex

History of Present Illness:Admitted for right upper quadrant abdominal pain lasting over 10 days.

Past Medical History:Chronic Hepatitis B

Laboratory Tests:HBV-DNA<1.00E+02  IU/ml , AFP  >1000 IU/ml,Alanine aminotransferase 23 U/L, Total bilirubin 9 μmol/L

Imaging Examination:Preoperative contrast-enhanced CT shows multiple intrahepatic space-occupying lesions.

Surgical Plan:

Following MDT consultation, a liver biopsy was performed to confirm the pathological diagnosis, followed by transarterial chemoembolization (TACE) for further treatment to control tumor progression.




Surgical Procedure



`Percutaneous Right Femoral Artery``Puncture`,selective angiography of the celiac artery and superior mesenteric artery demonstrated: an irregular vascular mass and tumor staining were observed in the right hepatic lobe, measuring approximately 6.0 × 7.0 cm, and an irregular vascular mass and tumor staining were observed in the left hepatic lobe, measuring approximately 3.0 × 3.0 cm。

Using a microcatheter and microwire, superselective catheterization was performed in the tumor-feeding arteries of the right and left hepatic arteries, respectively. Subsequently, 6 mL of iodized oil, 4 mg of raltitrexed combined with 1 g of Vispearl radiopaque drug-eluting microspheres (300–500 μm), and dozens of gelatin sponge particles were slowly injected into each artery.

Repeat angiography after embolization demonstrated the disappearance of tumor blush.




Postoperative Follow-up



Imaging at 1.5 months postoperatively showed no significant tumor activity, and the therapeutic response was evaluated as CR according to mRECIST criteria.
