On November 17, 2021, AbbVie announced that the European Commission (EC) had approved the second indication for Skyrizi (risankizumab), an interleukin-23 (IL-23) inhibitor, as monotherapy or in combination with methotrexate (MTX), for the treatment of adult patients with active psoriatic arthritis who have had an inadequate response to or are intolerant to one or more disease-modifying antirheumatic drugs (DMARDs).

Psoriatic Arthritis is a heterogeneous, systemic inflammatory disease with hallmark manifestations across multiple systems, including the joints and skin. Inflammation produced by the immune system can lead to joint pain, fatigue, stiffness, and cause red, scaly rashes.

Skyrizi selectively blocks IL-23 by binding to the p19 subunit of IL-23. IL-23 is a cytokine involved in inflammatory processes and is considered to be associated with many chronic immune-mediated diseases, including psoriasis. Skyrizi has been approved by the U.S. FDA and the European Union for the treatment of psoriasis. It is also in Phase 3 clinical trials for the treatment of inflammatory diseases such as Crohn's disease and ulcerative colitis.

This approval is based on positive data from two randomized, double-blind, multicenter, placebo-controlled Phase 3 clinical trials. Both trials met their primary endpoints, with 57.3% and 51.3% of patients receiving Skyrizi achieving an ACR20 response (indicating at least a 20% reduction in disease severity compared to baseline) at week 24 of treatment, compared to 33.5% and 26.5% in the placebo groups (p<0.001). Additionally, the trials also achieved sequential secondary endpoints, showing significant improvements in skin clearance (measured by PASI 90), physical function (assessed using the HAQ-DI index), and minimal disease activity (MDA).
