On March 23, 2022, Pfizer announced positive topline results from a Phase 3 clinical trial of etrasimod (ADP334). Etrasimod is an investigational, once-daily oral selective sphingosine 1-phosphate (S1P) receptor modulator being developed for the treatment of moderately to severely active ulcerative colitis (UC). The trial met its primary endpoint, showing statistically significant clinical remission improvement at week 12 in patients treated with etrasimod compared to placebo. Additionally, the trial demonstrated significant improvements across all key secondary endpoints.

UC is a chronic, often debilitating inflammatory bowel disease. Symptoms of the disease may include chronic diarrhea with blood and mucus, abdominal pain and cramping, and weight loss, significantly impacting patients' work, family, and social activities. Currently, the main treatment for the disease focuses on suppressing intestinal inflammation. However, existing treatments such as aminosalicylates, glucocorticoids, immunosuppressants, or biologics have shortcomings in terms of safety and efficacy. With continuous advancements in basic research on the immune system, new therapeutic targets related to the mechanisms of inflammation are constantly being discovered, and various novel targeted drugs have gradually begun to be applied in treatment. Among them, S1P receptor modulators are a current hotspot in research and development.

Etrasimod was developed by Arena Pharmaceuticals, which was acquired by Pfizer in December of last year. Etrasimod is a next-generation oral S1P modulator that specifically binds to S1P receptors 1, 4, and 5, potentially offering improved efficacy and safety profiles. Currently, this investigational therapy is being studied for a range of immune-inflammatory diseases, including UC, Crohn's disease, atopic dermatitis, eosinophilic esophagitis, and alopecia areata. Notably, Everest Medicines holds the development rights for etrasimod in Greater China.

This multi-center, randomized, double-blind, placebo-controlled Phase 3 clinical trial enrolled 354 UC patients who had previously failed or were intolerant to at least one conventional therapy/biologic/JAK treatment.
