Phase 3 PERSEUS Trial Shows 84.3% Four-Year Progression-Free Survival with Janssen/Genmab's Darzalex Faspro-Based Combination Therapy in Newly Diagnosed Multiple Myeloma

Johnson & Johnson announced today that the induction, consolidation, and maintenance treatment regimen based on the injectable anti-CD38 antibody therapy Darzalex Faspro achieved positive results in the Phase 3 clinical trial PERSEUS. In treating newly diagnosed multiple myeloma (NDMM) patients suitable for transplantation,This treatment regimen reduced the risk of disease progression or death by 58% compared to the standard therapy.Detailed data were presented as a breakthrough oral report at the 2023 American Society of Hematology (ASH) Annual Meeting and simultaneously published in The New England Journal of Medicine.

The trial results showed that using Darzalex Faspro in combination with bortezomib, lenalidomide, and dexamethasone (D-VRd) for induction and consolidation therapy, followed by maintenance therapy with Darzalex Faspro and lenalidomide (D-R), reduced the risk of disease progression or death by 58% (HR=0.42; 95% CI, 0.30-0.59; P<0.0001) compared to using bortezomib, lenalidomide, and dexamethasone (VRd) alone followed by lenalidomide (R) maintenance therapy. The quadruplet regimen also significantly improved the depth of response compared to VRd, including higher rates of complete response (CR) or better stringent complete response (sCR) and minimal residual disease (MRD)-negativity.

The estimated 48-month progression-free survival rate for the D-VRd regimen is 84.3%, compared to 67.7% for VRd.Compared with VRd, D-VRd treatment also achieved deeper remission, includingHigher sCR rate (69.3% vs 44.6%; P<0.0001). Higher overall MRD negativity rate (75.2% vs 47.5%; P<0.0001), and the sustained MRD negativity rate (≥12 months) in the D-VRd group was more than double that of the VRd group (64.8% vs 29.7%; P<0.0001).Although the overall survival (OS) data is not yet mature, the D-VRd group currently shows a favorable trend compared to the VRd group.

The overall safety of D-VRd is consistent with the known safety profiles of Darzalex and VRd. The most common Grade 3/4 hematologic and non-hematologic adverse events (AEs) for D-VRd versus VRd include neutropenia (62.1% vs 51.0%), thrombocytopenia (29.1% vs 17.3%), diarrhea (10.5% vs 7.8%), pneumonia (10.5% vs 6.1%), and febrile neutropenia (9.4% vs 10.1%).

Darzalex Faspro, jointly developed by Johnson & Johnson and Genmab, is the first anti-CD38 antibody that can be administered via subcutaneous injection, reducing treatment time for patients from several hours to a few minutes. In addition to receiving FDA approval for the treatment of multiple myeloma, it has also been approved for the treatment of immunoglobulin light chain amyloidosis.
