Case Brief
”

Patient:Female, 59 years old
Chief Complaint:Swelling and pain in the left lower limb for 4 days
Past Medical History:A 10-year history of diabetes; conservative treatment for "cerebral hemorrhage" at another hospital 4 years ago; cerebral infarction caused by occlusion or stenosis of the anterior cerebral artery 20 days ago, underwent interventional thrombectomy and carotid artery stent placement (details unknown).
Auxiliary Examination:
Ultrasound of the veins in both lower limbs showed: 1. Deep vein thrombosis in the left lower limb; 2. Thrombosis in multiple intramuscular veins of the left lower limb; 3. Varicose veins in the calf segment of the great saphenous vein of the left lower limb; 4. Dilation of a perforating vein 2 cm above the inner ankle of the left lower limb; 5. Subcutaneous edema in the left lower limb.
Lower extremity venography shows: contrast filling defects throughout the left popliteal vein, femoral vein, and iliac vein, thrombosis formed; thrombosis also found in the distal inferior vena cava.
D-Dimer Test: 6.52 ug/ml.

Treatment Strategy
”

Lower extremity venography of the patient showed: contrast agent filling defects throughout the left popliteal vein, femoral vein, and iliac vein, thrombosis formed, and thrombosis in the distal inferior vena cava.
Initial placement of inferior vena cava filter, followed by scheduled thrombectomy of the left lower extremity deep veins, left iliac vein, and inferior vena cava.

Surgical Procedure
”

The patient was in the supine position. The puncture site was slightly lateral to the pulse of the popliteal artery at the left popliteal fossa. Local infiltration anesthesia was performed with 1% lidocaine. Under ultrasound guidance, a left popliteal venous access was established, and a 12F sheath was inserted. Angiography showed contrast defects throughout the popliteal vein, femoral vein, and iliac vein, indicating thrombosis, as well as thrombosis in the distal inferior vena cava.



Under the guidance of a guidewire, a 12F thrombus aspiration catheter was placed in the inferior vena cava. The catheter was repeatedly withdrawn while performing thrombus aspiration, extracting a large amount of fresh thrombus.




Postoperative angiography showed that the blood flow of the popliteal vein, femoral vein, and iliac vein was unobstructed, with a small amount of residual mural thrombus in the common iliac vein. The blood flow of the inferior vena cava was unobstructed.



The puncture site was dressed with gauze, with an intraoperative blood loss of approximately 350ml, and the procedure went smoothly.

Postoperative Treatment
”

Anticoagulation, antiplatelet aggregation, stabilization of atherosclerotic plaques, and blood pressure control.
Oral Medications: Rivaroxaban Tablets 15mg Twice Daily with Meals; Clopidogrel Bisulfate Tablets 75mg Once Daily; Atorvastatin Calcium Tablets 40mg Once Daily
Regular复查, 1 time/month.

1992-1996: Medical Doctor (Ph.D.) in Cardiovascular Surgery at Shiga University of Medical Science, Japan; 1996-1998: Postdoctoral Fellow in Cardiac Surgery/Thoracic Surgery at the First Hospital of China Medical University; 2000-2002: Clinical Fellow (STA Fellow) in Adult Cardiac Surgery at the National Cerebral and Cardiovascular Center, Japan; 2004-2005: Clinical Fellow (DAAD Fellow) in Cardiac and Great Vessel Surgery at the German Heart Center.
Worked in cardiovascular surgery at the Thoracic and Cardiovascular Surgery Department of Yanbian University Affiliated Hospital, the First Affiliated Hospital of China Medical University, the First Affiliated Hospital of Jilin University, and the Fifth Affiliated Hospital of Zhengzhou University.

As the first investigator, received three third prizes for provincial and ministerial level scientific and technological progress awards, two second prizes for scientific and technological progress awards as the second investigator, and one second prize for Shenyang City scientific and technological progress award as the first investigator. During this period, published 97 papers in core journals and SCI papers as the first or corresponding author.
Department Introduction
Cardiovascular Surgery Department II focuses on the diagnosis and treatment of common and frequently-occurring diseases in the cardiovascular field in the Yanbian region, aiming to improve the diagnosis and treatment level of difficult cases. It is a newly established clinical department in September 2023, led by Professor An Jun, a second-level professor and chief physician in cardiovascular surgery. The department is a master's degree granting point in cardiovascular surgery, with five doctors currently: one professor, one attending physician, two resident physicians, and two master's students currently enrolled. An Jun, the "leading figure" of the department, was selected as a master’s supervisor (China Medical University) in 1997 and as a doctoral supervisor (Jilin University) in 2008. The needs of patients are our ongoing pursuit. The department will closely follow the development trends of cardiovascular surgery both domestically and internationally, continuously enhance its capabilities, unite efforts, and move forward with strength, enabling patients with cardiovascular diseases in the Yanbian region to enjoy high-quality medical services at their doorstep.
