This report is based on the results of an investigator-initiated exploratory clinical study (NCT04561557) evaluating the safety and efficacy of infusing Icaritin Cell Injection for the treatment of relapsed/refractory antibody-mediated idiopathic inflammatory neurological diseases. The study is being conducted at Tongji Hospital, affiliated with Tongji Medical College of Huazhong University of Science and Technology, by a research team led by Professor Wei Wang from the Department of Neurology at Tongji Hospital.
The study enrolled 12 subjects with aquaporin-4 (AQP4) antibody-positive relapsing/refractory neuromyelitis optica spectrum disorder (NMOSD). The enrolled subjects were patients who had been treated with at least one immunosuppressant for more than a year but had poor symptom control. The results showed that the Icarus injection demonstrated good tolerability and safety, persistent clearance of pathogenic antibodies, and potential clinical efficacy in NMOSD. CAR-T cells with chemotactic properties can penetrate the blood-brain barrier into the central nervous system, directly killing abnormal plasma cells, reducing the secretion of intrathecal autoantibodies and the abnormal activation of immune cells, thereby correcting the immunological disorder in the central nervous system of NMOSD patients, which is beneficial for the central immune reconstruction of patients.

The principal investigator of this clinical study, Professor Wang Wei from Tongji Hospital, Tongji Medical College, Huazhong University of Science and Technology"This study is the world's first clinical trial of CAR-T therapy in the field of AQP4-mediated relapsing/refractory NMOSD, not only demonstrating the encouraging efficacy and controllable safety of Icaritamab Injection in NMOSD but also elucidating the cellular kinetics and immunological characteristics of CAR-T therapy for central nervous system autoimmune diseases. It provides a new therapeutic approach for antibody-mediated autoimmune diseases and offers important scientific evidence for subsequent product iteration and improvement of CAR-T cell therapy for immune disorders. In addition to NMOSD, Icaritamab Injection has shown excellent clinical efficacy in treating other antibody-mediated autoimmune diseases, including myasthenia gravis and immune-mediated necrotizing myopathy, even reversing disease symptoms, with multiple related research papers published in authoritative international academic journals. The patient population with autoimmune diseases is vast, with recurring episodes that are difficult to cure, requiring long-term or even lifelong medication, representing a significant unmet clinical need.
