This report is based on the results of an investigator-initiated exploratory clinical study (NCT04561557) evaluating the safety and efficacy of infusing Icaritin Cell Injection for the treatment of relapsed/refractory antibody-mediated idiopathic inflammatory neurological diseases. The study is being conducted at Tongji Hospital, affiliated with Tongji Medical College of Huazhong University of Science and Technology, by a research team led by Professor Wei Wang from the Department of Neurology at Tongji Hospital.
The study enrolled 12 subjects with aquaporin-4 (AQP4) antibody-positive relapsing/refractory neuromyelitis optica spectrum disorder (NMOSD). The enrolled subjects were patients who had been treated with at least one immunosuppressant for more than a year but had poor symptom control. The results showed that the Icarus injection demonstrated good tolerability and safety, persistent clearance of pathogenic antibodies, and potential clinical efficacy in NMOSD. CAR-T cells with chemotactic properties can penetrate the blood-brain barrier into the central nervous system, directly killing abnormal plasma cells, reducing intrathecal autoantibody secretion and abnormal immune cell activation, thereby correcting the immunological disorder in the central nervous system of NMOSD patients, which is beneficial for central immune reconstitution in patients.
Tongji Hospital affiliated with Tongji Medical College of Huazhong University of Science and Technology and IASO Bio, as one of the world's first research teams to use BCMA CAR-T in treating immune disorders, have validated the clinical value of innovative cell therapy for relapsed and refractory immune diseases through their groundbreaking research achievements. This has increasingly gained recognition and attention globally. Currently,BCMA CAR-T therapy has been evaluated by the International Neuromyelitis Optica Study Group (NEMOS) as one of the most promising treatment strategies and included in the latest revised treatment recommendations (J Neurol 2024).Maximilian F. Konig, a professor in the Department of Immunology at Johns Hopkins University, pointed out in his Year in Review article for *Nat Rev Rheumatol* ("Immunotherapies in 2023: The Rise of Precision Cellular Therapies") that,The application of CAR-T therapy has brought about epoch-making progress in innovative cellular treatments for immunological diseases.。
The principal investigator of this clinical study, Professor Wang Wei from Tongji Hospital, Tongji Medical College, Huazhong University of Science and Technology"This study is the world's first clinical trial of CAR-T therapy in the field of AQP4-mediated relapsing/refractory NMOSD, not only demonstrating the encouraging efficacy and controllable safety of Ixcellen injection in NMOSD but also elucidating the cellular kinetics and immunological characteristics of CAR-T therapy in treating central nervous system autoimmune diseases. It provides a new therapeutic approach for antibody-mediated autoimmune diseases and offers important scientific evidence for the iterative improvement of subsequent CAR-T cell therapy products for immune disorders. In addition to NMOSD, Ixcellen injection has shown excellent clinical efficacy in treating other antibody-mediated autoimmune diseases, including myasthenia gravis and immune-mediated necrotizing myopathy, even reversing disease symptoms, with multiple related research papers published in authoritative international academic journals. The patient population with autoimmune diseases is large, with recurrent episodes that are difficult to cure, requiring long-term or even lifelong medication, reflecting significant unmet clinical needs. We will continue to explore the application of Ixcellen injection in more refractory autoimmune diseases in collaboration with the IASO Bio team, aiming to transform the treatment landscape of autoimmune diseases."
