B-type natriuretic peptide is an endogenous polypeptide secreted by the human body, and its production increases significantly as a compensatory mechanism in response to heart failure induced by pathological causes. As a sterile lyophilized preparation produced using recombinant DNA technology in Escherichia coli, it shares the same amino acid sequence as the endogenous brain natriuretic peptide generated by ventricular muscle.
The main mechanism of action is:
(1) B-type natriuretic peptide binds to specific natriuretic peptide receptors (which are coupled with guanylate cyclase), leading to an increase in intracellular cyclic guanosine monophosphate (cGMP) concentration and relaxation of smooth muscle cells. As a second messenger, cGMP can dilate arteries and veins, rapidly reducing systemic arterial pressure, right atrial pressure, and pulmonary capillary wedge pressure, thereby decreasing the preload and afterload on the heart and quickly alleviating the degree of dyspnea and systemic symptoms in patients with heart failure.
(2) B-type natriuretic peptide is a natural antagonist of the renin-angiotensin-aldosterone system (RAAS). It can antagonize endothelin, norepinephrine, and aldosterone in myocardial cells, cardiac fibroblasts, and vascular smooth muscle cells. It increases the glomerular filtration rate, enhances sodium excretion, reduces renin and aldosterone secretion, and also counteracts the sodium and water retention and blood pressure-raising effects of vasopressin and the sympathetic nervous system.
