The patient is a 78-year-old female who presented with progressive bilateral vision loss. Her corrected visual acuity was 20/100 in the right eye and 20/30 in the left eye. Ophthalmic examination of the right eye revealed changes in the anterior capsule of the lens.Lamellar Splitting, with nuclear cataract. Slit-lamp biomicroscopy revealed the anterior capsule of the lens in the nasal inferior quadrant of the right eye.LamellaPeeling floats in the anterior chamber (Fig. 1A); usingUltra-Wide AngleAnterior SegmentOCTShooting16mmWide、14mmUltra-deepHigh-definitionB-scanImageTo further clarifyAnterior SegmentInternal structural details,CaptureToExisting inAnterior ChamberIn、Attached to the anterior lensSuperficial ribbon-like capsuleStructure (Figure1B). The patient was diagnosed withTrue NatureCapsuleExfoliation Syndrome, followed by cataract extraction surgery on the right eye, which proceeded smoothly. On the 7th day post-operation during the follow-up examination, the corrected vision of the right eye recovered to 20/20.
