A total of 114 drugs were newly added this time, including 50 Class 1 innovative drugs and 7 traditional Chinese medicine varieties, with an overall success rate of 88%, significantly higher than the 76% in 2024. Additionally, 29 drugs that have been clinically replaced or have not been produced or supplied for a long time were removed. The new directory will be implemented nationwide in China starting from January 1, 2026.

After this adjustment, the total number of drugs in the basic medical insurance catalog has reached 3,253. For negotiated drugs within the agreement period that were not successfully renewed and have been removed from the catalog in this adjustment, a six-month transition period will be granted to ensure continuity of medication use.

High-priced drugs have always been the focus of adjustments to the medical insurance catalog. Previously, CAR-T therapy costing millions per dose was out of reach for ordinary patients due to its high cost. This time, there is a turning point in the medical insurance adjustment, with five CAR-T drugs passing the preliminary review and entering the commercial insurance innovative drug directory, breaking the deadlock in the high-priced drug market.

Among them, Fosun Pharmaceutical priced at 1.2 million RMB/bagAxicabtagene Ciloleucel, which has been applied for inclusion in the national medical insurance catalog for four consecutive years. It passed the formal review in both 2021 and 2023 but ultimately failed to enter the catalog. This achievement is overall fruitful, but there are still highly anticipated varieties that did not make it into the medical insurance or commercial insurance catalogs. For example,EisaiTheLemborexant Tablets、Simcere PharmaceuticalTheDaridorexant HydrochlorideAlthough passed the formal review of the 2025 National Medical Insurance and Commercial Insurance Innovative Drug List, it did not make it into the final basic medical insurance or the first edition of the commercial insurance innovative drug list.
The number of drugs in China's national medical insurance catalog has increased to 3,253. For innovative drugs successfully included in the catalog, the transition from "astronomical prices" to "affordable costs" has been achieved through a "volume-for-price" strategy. Five CAR-T therapies and multiple drugs for Alzheimer's disease (AD) and rare diseases have entered commercial insurance, breaking the deadlock for high-cost drugs while also broadening the coverage for medications treating special diseases. Despite the overall fruitful results this time, some highly anticipated drug varieties did not make the cut, highlighting the strictness of the medical insurance adjustment process. In the future, continuous adjustments to medical insurance policies will further promote industry innovation and development, benefit people's livelihoods, and safeguard patients' health.
