Recently,Boston Scientific announced the acquisition of two anastomotic leak solution products from B. Braun Medical. These two products are respectivelyEso-SPONGE(Products used for the treatment of upper gastrointestinal anastomotic leaks and perforations) andEndo-SPONGE(Used for treating colorectal anastomotic leakage).
It is reported that Eso-SPONGE is an endoluminal vacuum therapy (E-VAC) technology made from porous polyurethane sponge. The polyurethane sponge is connected to a 100 cm long drainage tube at the end, used for treating anastomotic insufficiency or perforations in the upper gastrointestinal tract.
This acquisition of the B. Braun’s Endoscopic Vacuum Therapy (E-VAC) technology by Boston Scientific not only enriches its therapeutic product line under digestive endoscopy but also expands its business into new areas.
B. Braun Medical E-VAC TechnologyAddressing Industry Pain PointsData shows,Anastomotic leakage of the upper and lower digestive tract is a serious postoperative complication.According to clinical statistics, the incidence of anastomotic leakage after esophagectomy is as high as 30%, and the associated morbidity and mortality are significant.Up to 19% of patients are diagnosed with anastomotic leakage after colorectal surgery. The complications of these two surgeries are one of the biggest challenges currently faced by clinicians.Currently, the treatment for anastomotic leakage in the lower digestive tract mainly involves antibiotics, rectal irrigation, and drainage. If these methods cannot effectively control the situation, debridement and permanent stoma surgery are required; however, there are relatively fewer solutions for anastomotic leakage in the upper digestive tract.
Faced with this dilemma, B. Braun developed Endo-Vacuum Assisted Closure (E-VAC) technology in the early 21st century, initially applying it for the treatment and prevention of anastomotic leakage in the digestive tract. Subsequently, B. Braun extended the use of this technology to the treatment and prevention of upper gastrointestinal anastomotic leakage.B. Braun discovered through clinical research that Endoluminal Vacuum Therapy (E-VAC) technology not only effectively prevents anastomotic leakage but also accelerates healing and avoids secondary surgery if anastomotic leakage occurs.
The acquisition of the E-VAC (Endoscopic Vacuum Therapy) technology from B. Braun by Boston Scientific not only enriches its therapeutic product line under digestive endoscopy but also expands its business into new areas.
Two Products for Gastrointestinal Anastomotic LeakagePublic information shows that Eso-SPONGE and Endo-SPONGE are products in the B. Braun Medical portfolio that utilize Endoluminal Vacuum Therapy (E-VAC) technology for the treatment of gastrointestinal anastomotic leaks.
Eso-SPONGE
Eso-SPONGE is an Endoluminal Vacuum Therapy (E-VAC) technology used to treat anastomotic insufficiencies or perforations in the upper gastrointestinal tract.Made from porous polyurethane sponge, the polyurethane sponge is connected to a 100 cm long drainage tube at the end. It is designed to be installed in a cavity and, when connected to a vacuum pump, can drain, clean, and reduce the cavity.

Eso-SPONGE Treatment Principle: When the polyurethane sponge is implanted into the patient's body, effective continuous drainage can be achieved by applying negative pressure, continuously draining fluid from the cavity, controlling infection, cleaning the cavity (removing fibrin), promoting granulation of healthy tissue, and mechanically reducing the size of the cavity.
Eso-SPONGE has been on the market since 2014,A large amount of clinical research data has proven that the use of Eso-SPONGE for treating upper gastrointestinal anastomotic leakage can effectively promote the healing of the anastomosis.
Endo-SPONGE
Endo-SPONGE, like Eso-SPONGE, is an intraluminal vacuum therapy (E-VAC) technology, but with a different intended use. Endo-SPONGE is used to treat anastomotic insufficiency or perforations in the lower gastrointestinal tract. Its composition is similar to that of Eso-SPONGE, made from porous polyurethane sponge. A 100 cm long drainage tube is connected to the tail of the polyurethane sponge. It is designed to be installed in a cavity and, when connected to a vacuum pump, can drain, clean, and reduce the cavity.
In 2008, Endo-SPONGE was first used for the treatment of rectal anastomotic leakage, and more than 40 clinical research papers have been published to date.
Based on this, Eso-SPONGE and Endo-SPONGE also have their prominent advantages in treatment. Compared with traditional treatment options, the former two can continuously drain infected wound fluid from the cavity, and compared with irrigation,The treatment time for the new technology has been significantly shortened, with a median treatment duration of 20-50 days, and the anastomotic leak closure rate is as high as 67-100%.
Global Endoscopy Market Continues to GrowPromote the Development of Endoscopic Consumables
According to statistics, the global endoscopic devices market size reached USD 20.2 billion in 2019, with a compound annual growth rate (CAGR) of 5.4% from 2015 to 2019. It is projected that by 2024, the total scale of the global endoscopic devices market will grow to USD 26.98 billion, with a CAGR of 6.0% from 2019 to 2024.Endoscopy, as a minimally invasive surgical and interventional treatment device in the new type of medical precision diagnosis and treatment equipment, has a history of more than 200 years in the field of medical endoscopy. Currently, the global endoscopic diagnosis and treatment device market is experiencing steady growth with a high level of market concentration. Among them,Boston Scientific and Olympus together account for 70% of the market share, with Boston Scientific's market share reaching 36%.It is reported that,Boston Scientific's endoscopic diagnosis and treatment device product line is complete, covering basic types, EMR/ESD types, dilation types, EUS types, ERCP types, etc.Among them, ERCP devices and stents applied in the field of biliary and pancreatic diagnosis and treatment are the core products of the company's endoscopic diagnostic and therapeutic devices. In 2018, the global market size for biliary and pancreatic endoscopic devices was $1.4 billion, and at that time, Boston Scientific's market share exceeded 60%.Currently, Boston Scientific has completed the full-line layout of "disposable duodenoscope + cholangioscope + supporting diagnostic and therapeutic devices," establishing a wide moat. Leveraging disposable endoscopes, the company is expected to transition from a single endoscopic therapeutic device business to a "flexible endoscope + endoscopic therapeutic device" model, opening up broader development opportunities. The development of endoscopic surgery has promoted the sales of consumables under endoscopy. As early as 1966, Olympus began developing endoscopic tissue biopsies. The subsequent development of ERCP significantly expanded the application scope of endoscopic surgery. Later, the development of ESD/EMR became representative of the third phase of endoscopic diagnosis and treatment methods. The advancement of these techniques has provided patients with minimally invasive, cost-effective treatments that offer a high quality of life post-procedure.An effective alternative to traditional open surgery, the development of endoscopic surgery has directly driven the expansion of varieties and sales growth of endoscopic diagnosis and treatment consumables.Endoscopic consumables, as one of the fastest-growing sub-sectors in the global medical device industry,Boston Scientific has also long been among the top three in the field of endoscopic diagnosis and treatment for pancreaticobiliary diseases, gastrointestinal cancer, and gastrointestinal bleeding. So, this time fromB. Braun Medical Acquires Two Anastomotic Leak Solution ProductsLater, what kind of development will Boston Scientific face in this field? Instrument Family will continue to follow up.
