The successful completion of the surgery not only marks the formal entry of this innovative domestically produced PFA system into the clinical application stage, but also provides a better option for interventional treatment of atrial fibrillation with its three core advantages: integrated mapping and modeling with ring catheters, efficient ablation, and excellent safety.

Case Summary

The patient is a 70-year-old female who previously underwent pulmonary vein isolation (PVI) ablation using radiofrequency (RF) energy, followed by recurrence of paroxysmal atrial fibrillation. Based on preoperative echocardiography, which showed a left atrial diameter of 30 mm, the operator selected the AccuPulse 15–20 mm adjustable loop cardiac pulsed field ablation catheter for the procedure.

Preoperative Substrate Mapping Map

Preoperative substrate mapping revealed a distinct ablation line with no detectable potentials in the left pulmonary veins, whereas electrical activity had recovered in the right pulmonary veins. Therefore, this procedure focused on supplemental ablation targeting the right pulmonary veins. The total procedure time was only 67 minutes, including 13 minutes for modeling and mapping. Pulmonary vein isolation (PVI) using the PFA catheter was completed in just 20 minutes. A total of 35 PFA ablations were delivered, with a cumulative energy delivery time of only 6.125 seconds.

Comparison of Potentials Before and After RSPV Ablation

The operator adopted an ablation strategy of “entering the pulmonary vein with a small-diameter catheter loop and then expanding the loop diameter,” delivering radiofrequency energy only after confirming good contact force indicators. A total of four energy delivery sets (23 applications) were administered to the right superior pulmonary vein (RSPV). At each position, six ablations were performed, followed by rotation through a specified angle (e.g., 180°) and another six ablations to consolidate the lesion set and address potential gaps. Subsequently, 11 ablations were delivered slightly deeper at the pulmonary vein ostium, followed by 12 ablations at the vestibular region upon withdrawal, achieving continuous coverage from deep to superficial layers. For the right inferior pulmonary vein (RIPV), two energy delivery sets were applied, with 12 ablations performed at the vestibular region. After completion of ablation for all four pulmonary veins, electrophysiological testing and repeat substrate mapping were conducted to confirm complete pulmonary vein isolation, and the procedure was concluded successfully.

The operator’s team evaluation indicated that the ablation procedure proceeded smoothly throughout, with no serious adverse events occurring intraoperatively. The patient recovered well postoperatively, without any complications. AccuPulse’s adjustable circular PFA catheter achieved rapid and effective ablation within the pulmonary veins.
