Recently,Affiliated Hospital of Nantong UniversityAs one of the first clinical application units in China,By Professor Lu Qi, Director of the Department of Cardiology, and his teamApplication of AccuPulse Medical®Disposable Cardiac Pulsed Field Ablation Catheter and AccuBlator®Cardiac Pulsed Field Ablation System Successfully Performed Cardiac Pulsed Field Ablation (PFA) Procedure on a Patient with Atrial Fibrillation.

The successful completion of the surgery not only marks the official entry of this innovative domestically produced PFA system into the clinical application stage, but also provides a better option for interventional treatment of atrial fibrillation with its three core advantages: integrated mapping and modeling with a circular catheter, efficient ablation, and excellent safety.

Case Summary

The patient is a 61-year-old male. Preoperative examination revealed an anteroposterior diameter of the left atrium of 47 mm. Intraoperative modeling using the HD Grid catheter indicated enlarged pulmonary veins. The operator employed the AccuPulse Medical 15–20 mm adjustable-loop cardiac pulsed field ablation catheter. The procedure focused on pulmonary vein isolation (PVI). To ensure the quality of PVI and reduce the risk of recurrence, additional ablation pulses were appropriately delivered to the regions with enlarged pulmonary veins, thereby enhancing the continuity of ablative lesions and ensuring effective isolation.

The total procedure time was 97 minutes, with only 9 minutes required from venous puncture to transseptal puncture for establishing left atrial access. Mapping and modeling took 15 minutes, and pulmonary vein ablation lasted 60 minutes, delivering a total of 216 ablation lesions. Intraoperatively, circumferential pulmonary vein isolation was performed on the left superior and right inferior pulmonary veins, among others, with an additional 12 supplemental ablations applied to the anterior-superior aspect of the right-sided veins. Following ablation, substrate mapping was conducted using the catheter, and bidirectional pulmonary vein block was verified under pharmacological provocation. After confirming the efficacy of the ablation, the catheter was withdrawn, and the procedure was completed successfully.

The operator fully affirmed the clinical performance of the catheter. The loop-adjustment function operated normally, with a stable and controllable circular configuration that rapidly adapted to the pulmonary vein antrum across varying anatomical structures. The overall manipulation process was smooth, and contact-force feedback was clear and sensitive, enabling the operator to accurately assess the degree of catheter-myocardial tissue contact and effectively ensure ablation efficacy. The impedance monitoring function was reliable, with stable real-time data providing robust support for the safety and effectiveness of energy delivery. In summary, the catheter features a rational design and reliable performance, meeting the requirements for circumferential pulmonary vein isolation under various complex anatomical conditions, including enlarged pulmonary veins, thereby providing strong assurance for the precise implementation of pulsed field ablation.
