Recently, during the grand opening of the 26th Cardiac Electrophysiology Conference, AccuPulse successfully held a special surgical demonstration session themed “Circular Intelligent Control, Precise Pulsing.”Under the leadership of President Zhou Yafeng,Director, Department of Cardiology, The Fourth Affiliated Hospital of Soochow UniversityWang Linlin and TeamSuccessful Application of AccuPulse Medical's AccuPulse®Disposable Cardiac Pulsed Field Ablation Catheter and AccuBlator®Cardiac Pulsed Field Ablation System: A cardiac pulsed field ablation (PFA) procedure was performed on a patient with atrial fibrillation.

The successful completion of the surgery not only marks the official entry of this innovative domestically produced PFA system into the clinical application phase, but also provides a superior option for interventional treatment of atrial fibrillation with its three core advantages: integrated mapping and modeling with a circular catheter, efficient ablation, and excellent safety.

Case Introduction

The patient was a 70-year-old male. Based on preoperative examinations, his left atrial diameter measured 47 mm. The operator successfully completed the procedure using the AccuPulse 20–25 mm adjustable-ring cardiac pulsed-field ablation catheter.

PVI Ablation Map

During this procedure, the operator performed circumferential pulmonary vein isolation. The time from venous puncture to completion of transseptal puncture was 5 minutes, mapping and modeling took 9 minutes, and the dwell time in the left atrium was 91 minutes. A total of 189 radiofrequency ablations were delivered across the pulmonary veins, with a cumulative energy delivery duration of 33.075 seconds. Ablation details for each pulmonary vein were as follows: LSPV 42 applications (24 at the ostium, 18 at the antrum), LIPV 57 applications (30 at the ostium, 27 at the antrum), RSPV 36 applications (30 at the ostium, 6 at the antrum), and RIPV 54 applications (24 at the ostium, 30 at the antrum). During ablation of the left superior pulmonary vein, the patient’s atrial fibrillation spontaneously converted to sinus rhythm.

Pre- and Post-operative Substrate Mapping

Following the completion of ablation, the operator performed substrate mapping using the Accupulse catheter. The results indicated favorable left atrial substrate, and the isolation coverage of the four pulmonary veins and their antra was also satisfactory. The procedure was concluded successfully.

During the procedure, the AccuPulse adjustable-loop PFA catheter demonstrated excellent performance in mapping and modeling. The catheter achieved precise anatomical modeling, successfully completing the 3D reconstruction of the left atrium and pulmonary veins. Substrate mapping yielded high-quality results, providing clear and reliable navigation support for the surgery. Throughout the procedure, catheter maneuverability was smooth, resulting in a satisfactory operator experience. During the ablation phase, the catheter achieved efficient pulmonary vein isolation (PVI). Ablation at the ostia and antra of all four pulmonary veins was completed with a cumulative total of 189 energy deliveries. The flexible and controllable discharge strategy reflected a robust design foundation. Overall, this ablation system is safe, controllable, and excels in anatomical modeling, making it a reliable tool for pulse field ablation in atrial fibrillation.
