Electronic Referral (E-referral) is an electronic platform that enables the seamless transmission of patient information from primary to secondary care customer management systems. E-referral is rapidly replacing paper-based referrals and is making swift progress toward the ultimate goal of achieving seamless communication and information sharing among healthcare institutions. Widespread adoption of electronic referral systems can achieve three major objectives: improved individualized care, enhanced population health, and reduced costs.

Advantages of E-Referral

From the patient’s perspective, e-referral provides accurate and timely referral services, enhancing the coordination of subsequent treatment and care. With access to patients’ complete diagnostic and treatment information, hospitals and physicians can devise more effective treatment plans. The sharing of patient information across various healthcare institutions eliminates the need for redundant tests. As delays associated with the transmission of paper-based medical records are mitigated, patients experience shorter waiting times and faster consultation processes. Furthermore, the enhanced security of information transmission in electronic referrals offers better protection of patient privacy.From the healthcare institution’s perspective, digitization has resolved issues such as illegible handwriting on paper documents and poor quality of faxed files, significantly improving the quality of medical records related to patients. Finland was the first country to adopt an electronic referral system, with its capital, Helsinki, introducing it in 1990. The e-referral initiative enabled more patients to receive treatment and improved the healthcare system. Initially involving only 200 general practitioners and a population of one million, the project saw a total of 67,000 e-referrals processed by 2002 after twelve years of implementation.

Denmark was the second country to implement an electronic referral system. In 1995, the electronic referral system was introduced by a national MedCom project known as The Danish Health Care Data Network. The TeleMed project aimed to strengthen electronic communication among various healthcare service providers, with electronic referrals serving as a formal component of this initiative. By 2004, 41% of all referrals in Denmark were conducted electronically. Electronic referrals have played a significant role in improving treatment outcomes, quality of care, and medical efficiency within the healthcare sector.

In 2007, the Hutt District in New Zealand launched an electronic referral project. The initiative aimed to provide general practitioners with complete electronic referral information for patients, thereby facilitating timely and effective clinical treatment decisions. By 2009, 90% of referrals were conducted electronically. The Hutt District has been recognized by the New Zealand Ministry of Health as a “leader” in electronic referral systems.

In 2005, San Francisco General Hospital (SFGH) began providing specialized care services to patients. However, there was a severe mismatch between the supply and demand for these services, with patients typically waiting 7–11 months for a routine clinical appointment, significantly delaying treatment. To address the need for timely care and appropriate patient triage, the hospital implemented an internet-based electronic referral system that integrates specialist referrals and consultative advice. The e-referral system has not only facilitated better communication between physicians and patients but also helped the hospital optimize the allocation of specialist resources and expand primary care capacity.

With the rapid development of the Internet in China, certain hospitals have begun to adopt electronic referral systems to enhance disease prevention and control capabilities, improve medical treatment services, and optimize the allocation of healthcare resources. Taking Shanghai Shibei Hospital as an example, it has established a networked electronic referral mechanism with community health service centers. When triage staff access the “Electronic Two-Way Referral” information system, relevant patient information and referral requests from community health service centers are immediately displayed on the screen. This allows triage personnel to clearly and accurately understand patient needs and required post-referral treatments before the patients arrive, enabling them to notify specialists in the relevant departments and promptly initiate preparatory work. Upon arrival at the hospital, patients bypass procedures such as queuing, registration, and payment, thereby receiving rapid diagnosis and treatment.
