The platform consists of core modules including diabetes risk assessment, doctor-patient communication, nutritional guidance, blood glucose monitoring, exercise management, medication reminders, online education, and data analysis. It features integration and collaborative functionality with hardware devices such as smart blood glucose meters and blood pressure monitors. The system provides convenient risk assessments for individuals at high risk of diabetes, offers comprehensive out-of-hospital management and support for diabetic patients, and assists physicians in making precise diagnoses. It is widely applied in hospital development and patient management.
