Kongzhuang Diabetes Partners with The First Affiliated Hospital of Jinan University to Manage 200,000 Diabetic Patients Through Internet-Based Platform

On April 20, with the launch of the “Internet + Diabetes Management Platform” at the First Affiliated Hospital of Jinan University in Guangzhou, the Diabetes Management Center of the First Affiliated Hospital of Jinan University, supported by Zhangkong Diabetes, and China’s first “Internet Diabetes Hospital,” were officially established in Guangzhou. This means that more patients with diabetes can benefit from professional, integrated diabetes management services both inside and outside the hospital.

At the meeting, Zhou Peiru, Deputy Director of the First Affiliated Hospital of Jinan University and project leader, stated, “Over the next three years, leveraging the Diabetes Management Platform and in conjunction with the advancement of the tiered diagnosis and treatment system, our nursing team will enroll 200,000 patients with diabetes into the hospital’s Diabetes Management Center for standardized care.”

Zhangkong Diabetes is a diabetes management service connectivity platform developed based on an intelligent data engine. Physicians and medical assistants collaborate on the platform to provide patients with personalized, continuous, and integrated diabetes management services both inside and outside the hospital. As a leader in China’s mobile healthcare sector for diabetes, Zhangkong Diabetes has long been committed to partnering with medical institutions, aligning with the trend of tiered diagnosis and treatment, and actively exploring more practical diabetes management models. On November 28, 2015, at the China Chronic Disease Conference, the Chinese Center for Disease Control and Prevention released the “Evaluation Report on the Effectiveness of the Zhangkong Diabetes Platform.” The report demonstrated that the services provided by the Zhangkong Diabetes Platform help improve patient adherence to blood glucose monitoring, regular medication use, and effective physical activity, thereby facilitating blood glucose control in diabetic patients. It is considered a feasible and appropriate technology for diabetes control. Zhangkong Diabetes is a partner of organizations including the Chinese Medical Doctor Association, Tencent Tang Daifu, Alibaba Future Hospital, and the First Affiliated Hospital of Xiamen University.

In the second half of 2015, the First Affiliated Hospital of Jinan University began managing patients with diabetic foot through the Zhangkong Diabetes platform. Through the joint efforts of the hospital and Zhangkong Diabetes, six months of management led to significant improvements in patients’ disease control, clinical outcomes, and adherence.

In March 2016, the First Affiliated Hospital of Jinan University, South China University of Technology, and Tangkong Diabetes jointly submitted the project “Intelligent Wearable Monitoring Devices for Diabetes and Cloud Platform for Complication Prevention and Control,” which was approved as a Major Scientific Research Special Project of Guangdong Province. Today, the hospital and Tangkong Diabetes have officially upgraded the platform to the “Diabetes Management Center of the First Affiliated Hospital of Jinan University.” Through various channels—including web portals, mobile apps, Alipay service windows, and hotlines—the center aims to explore and promote standardized diabetes management services across all departments of the hospital and the 12 community health service centers within the “Medical Consortium” under the tiered diagnosis and treatment system. This enables healthcare professionals to manage diabetic patients more effectively and comprehensively via the platform. Furthermore, given the high prevalence of diabetes across various departments, interdisciplinary collaboration and care are particularly crucial. The establishment of the “Diabetes Management Center of the First Affiliated Hospital of Jinan University” represents a breakthrough in this regard.
