On October 18, the expert consensus conference on “Construction and Application of a Graded Collaborative Prevention and Treatment Platform for Severe Diabetes Complications,” jointly convened by the Shanghai Municipal Science and Technology Commission and the Songjiang District Health and Family Planning Commission of Shanghai, was officially held at the Songjiang District Medical Association in Shanghai.

As a lifelong chronic condition, diabetes requires not only professional medical services from hospitals but also, more critically, effective daily medication management by patients. Despite the escalating disease burden of diabetes and substantial government investment in healthcare and public health resources, the situation has not seen significant improvement. Meanwhile, internet-based online platforms, driven by capital influx, have entered the competitive landscape. However, their development has been frequently constrained by issues such as insufficient understanding of medical care and low patient user stickiness. In light of these trends, integrating and leveraging multi-faceted social resources through public-private partnerships will undoubtedly bring new possibilities to the chronic disease management of diabetes.

At this conference, Kongzhang Diabetes, as the platform provider for the project, will deliver a suite of services to support coordinated prevention and treatment of severe diabetic complications in Songjiang District, including a “Patient Complications Database,” “Intelligent Assessment, Screening, and Tiered Diagnosis and Treatment Recommendations for Complications,” and “Tracking and Management of Patients with Complications.” These services will establish an innovative model for regional coordinated management of complications: hospitals at all levels in Songjiang District will use the platform to conduct complication screenings, generate assessment reports, and implement tiered diagnosis and treatment based on the assessment results.

In its collaboration with the Health Department of the Songjiang District Government, Zhangkong Diabetes (Diabetes Control) provides a support platform featuring standardized management processes that deliver intelligent reminders and decision-making recommendations. The system generates diagnostic and treatment alerts based on standardized protocols for diabetes complications, thereby enhancing the standardized clinical capabilities of physicians at all levels. Meanwhile, the Zhangkong Diabetes platform has established a “Home Blood Glucose Monitoring Database” and a “Diabetic Foot Screening Database” for Songjiang District. By enabling open data sharing, the platform ensures homogeneous medical practices within the Songjiang area, thus optimizing the allocation of medical resources for tiered diagnosis and treatment. Additionally, the platform has developed a patient management service system, forming a multi-dimensional service network dedicated to ensuring the accessibility and continuity of diabetes management.
