In September 2015, the State Council issued the Guiding Opinions on Promoting the Construction of a Tiered Diagnosis and Treatment System, clarifying that improving the tiered diagnosis and treatment service system should focus on strengthening primary care. By 2017, the policy framework for tiered diagnosis and treatment was gradually improved, with high-quality resources being systematically and effectively decentralized; by 2020, the service capacity for tiered diagnosis and treatment was comprehensively enhanced, safeguard mechanisms were progressively strengthened, and a tiered diagnosis and treatment model featuring initial consultation at the primary level, two-way referral, separate management of acute and chronic conditions, and coordination between upper- and lower-level institutions was gradually established.

Through telemedicine-based assistance, it strengthens the imaging, pathology, cardiology, and respiratory departments in these facilities, thereby enhancing their precision diagnosis and treatment capabilities and addressing the prevalent issues of inaccurate diagnosis and suboptimal therapy at the grassroots level.


Cloud-Based Medical Imaging Facilitates the Downward Distribution of High-Quality Healthcare Resources


The shortage of physicians and medical technologists, outdated diagnostic equipment, and the absence of information technology systems have become major challenges for primary healthcare institutions. As imaging examination is one of the most fundamental diagnostic services in hospitals, providing support to primary care facilities has become an urgent priority.


In September 2016, Medpacy, in collaboration with the Collaborative Innovation Center, Shanxi Medical University, North University of China, and Shanxi Unicom, launched the Shanxi Medical Imaging Cloud. This initiative provides cloud-based PACS services to primary care hospitals. Additionally, leveraging its physician group, Medpacy delivers remote imaging consultations and tele-education to grassroots medical facilities, thereby facilitating the effective decentralization of high-quality medical resources and establishing a tiered diagnosis and treatment model characterized by vertical integration between upper-level and primary care institutions.


Family Doctor Contract Services Implement Tiered Diagnosis and Treatment


In 2016, Shanxi Province issued the “Notice on Printing and Distributing the Shanxi Provincial Health Service System Plan (2016–2020),” which explicitly required the implementation of a tiered diagnosis and treatment system and family doctor contract services across all prefecture-level cities. This initiative aimed to further deepen healthcare reform, optimize the allocation and utilization of medical resources, and alleviate the public’s difficulties in accessing affordable and timely medical care.


How can we encourage the public to prioritize seeking care at nearby facilities when health issues arise, ensure their satisfaction with services provided by primary healthcare institutions, fundamentally transform the healthcare-seeking habits of residents within the jurisdiction, and truly achieve the desired outcome of “managing minor illnesses at the primary care level”?

General practitioners serve as a crucial supplement to the current primary care physician workforce in China. The severe shortage of general practitioners in Shanxi Province has led to sluggish progress in the implementation of family doctor contract services. It is essential to adhere to the key priorities of ensuring basic coverage, strengthening primary care, and establishing robust mechanisms.
